You can type this details in one Word document and format it, print the form in A4 sheet without signature. Once you took print out put your signature and put it in one envelop and front of the envelop mention subject and To address. submit it in HDFC bank or HDFC ATM post boxes. Within 4 to 5 days you will receive one call regarding cancellation confirmation. Once you confirm the same they will approve to cancel your card.
